Requirements
First: Service Eligibility Requirements
Geographical Scope:
The beneficiary must reside within the Eastern Province, and the requested service must fall within the association’s approved scope of services.
Identity Verification:
- National ID
- Resident ID (Iqama)
- Passport
- Valid visit visa
Financial Status Verification:
- Salary certificate attested by the Chamber of Commerce
- Social Insurance (GOSI) report
- Bank statement for the last three months
Medical Condition Verification:
A recent medical report or prescription not exceeding 3 months, issued by an accredited healthcare provider within the Kingdom.
If the beneficiary has medical insurance, documentation proving the insurer’s refusal to cover the treatment must be attached.
Proof of Residence:
- Property deed
- Valid lease contract
- Official letter from the employer
Second: Medical Services Policy
Coverage Scope:
The association provides medical services to beneficiaries who meet all eligibility criteria, with the exception of the following cases:
- Intensive care and long-term care
- Dental treatments (except life-threatening cases)
- Rehabilitation therapies (physical therapy, autism therapy, speech therapy, palliative care)
- Psychological support and mental health treatments
- Cosmetic surgeries (except for medically necessary functional procedures)
- Organ transplantation
- All childbirth cases
Third: Medication and Medical Devices Policy
1. Medication Dispensing Regulations:
- Only medical prescriptions issued within the past 3 months are accepted, and a new prescription is required once this period lapses.
- Prescriptions must be clear, aligned with the diagnosis, and issued by an accredited healthcare provider within the Kingdom.
- All prescriptions must be updated annually after the beneficiary’s application is approved.
For psychiatric and neurological medications:
- The prescription must be stamped by a specialized consultant.
- It must be updated every 6 months at most.
2. Medical Device Dispensing Regulations:
- A certified medical report must be provided, clearly indicating the beneficiary’s need for the device.
- Devices are selected based on the beneficiary’s actual needs and in accordance with approved quality standards.
- All devices undergo inspection and verification of functionality prior to delivery.
- The beneficiary must sign a device receipt form and commit to maintaining the device and returning it if it is no longer needed.
